Parent Forms

The district, school, and/or teacher may distribute various forms for electronic signature through this portal.

These include: the student/family handbook, district device insurance agreement, field trip forms for example

  • To Access forms in the Parent Portal log in to the PowerSchool Parent Portal

  • Once Logged In you will see Forms on the Side Bar

  • District and school-related forms will be located in the General Forms tab, and classroom-related forms will be located in the Class Forms tab.

  • Forms that need to be completed will show an icon. You can click on the title of the form to review the item to be signed.

  • On the next screen, you can review the item to be signed. Click the check box affirming that you have read and acknowledge the information it contains when ready. Finally, click the submit button to electronically sign the form.

  • A pop-up will appear to confirm that you submitted the form.

  • Back on the Forms page, the “Not Started” icon will no longer appear.
