How to create a form/permission slip
ParentSquare allows teachers to create forms for their classrooms, such as permission slips. In ParentSquare, you’re able to manage form data, send reminders, fill in forms on user’s behalf, and even sending reminders to those who need them. In this article, you will learn how to create a permission slip specifically, though the same concept can be applied to other forms.
Creating a Form in a Post
Begin to Create a New Post. Instructions on this can be found here. Do not finish posting your post yet.
Once your finished creating your post, click on Forms / Permissions on the left hand side bar.
You will be taken to a page to create your Form. First, you should set what date you would like this completed by, which you can set by clicking the date next to the When do you need completed form by field.
Notice the options for Select an existing form and Create a new form. ParentSquare and the district offer a selection of pre-made forms for use, such as basic Permission Slips, Media Releases, and Residence Verification to name a few. In majority of cases, you will want to select Create a new form. Once you click on this, you will see a few options.
Form Requires Signature: This form will require an eSignature from a parent or guardian. Currently, options are Permission, Approval, and Verification. By clicking Show Disclaimer, you can see exactly what question the parent is being asked.
Form can have Attachments: If your form is for something that requires an attachment, such as a physical or doctors note, you can select Form Can Have Attachment. There, you can select whether an attachment is Optional or Mandatory.
Require Student Information: By default, this is selected. On the Parent’s side, this information will be auto filled, meaning less possible points of failure or wrong information for a student. Student Information would include Name, Email, Phone Number, etc.
You can also enter additional questions at the bottom. Click on the Enter Question field to begin typing a question. You can choose which answer you would like back, such as Short Answer, Paragraph, Select One, Select Many, and Date. For Select One/Many, type each response on a seperate line, as seen below.
You can now click Post Now.
NOTE: Parents must sign their name on the form exactly as it’s displayed. For example, if a parent named John Smith signs the document as John A. Smith, the form will not be accepted.
Managing Form Data
Click the Gear in the top right corner of your post.
Hover over Forms, then select View Report
Here, you can see a report on all students that have had this form filled out.
Clicking the number above Awaiting Reply will show all students who have not yet submitting this form. We have two options here by each name:
If a parent prints out the form and hands it in, you can click Mark Completed, and the student will appear as though they completed the Electronic Permission Slip.
Another option we have is Fill Form, where you can fill out a form on a parent’s behalf. For example, if a parent prints and hands in a paper copy, you could also fill out the form so you have this data in ParentSquare.
Clicking the number above Completed will allow you to see any student’s completed form, and download them for your own records. Click Download to download this record.
