This article will show you how to enter in Health Conditions for students in the Synergy Health screen. The Health Conditions allow you to enter any ongoing health conditions a student may have.

Instructions

  1. In the Health screen, click on Health Conditions

  2. Then click +Add in the Conditions grid

  3. Click on the dropdown under Health Conditions in the grid and select a Condition from the list

  4. After the condition is selected, continue filling out the rest of the fields to complete the record

    1. The End Date field may or may not be blank, this can be entered in for conditions such as a broken leg or arm since they have “end dates”.

    2. Comments can be added, but are not required. However, these are very helpful for other Nurses to read and understand situations quickly.
